The following course has been APPROVED for Pennsylvania pesticide recertification training credits. When the training is completed return an attendance sheet or forms of your own design listing the Meeting number 000J11 and the attendees: Name, Birth date, Signature, and PA Certification Number. (If their certification number is not available please include their address.), OR You may use the attendance sheet available at http://www.pested.psu.edu/pdaprog/api-336.pdf

By submitting an attendance list for credit assignment you are certifying that this Pesticide Recertification Course was held as pre-approved, or have submitted an accompanying list all changes to the pre-approved course, and the persons were in attendance for the entire course.
Understand that submitting information for pesticide recertification is a legal process. Falsification by a course sponsor responsible for the information provided, and/or the training conducted may result in the withdrawal of course approval, not only for the submitted course, but also for future courses, and that the Department reserves the right to further legal remedies.
